Submit Financial Reports
Financial Reporting Forms
As a continued effort to improve our efiling system, the Department has revised the quarterly and annual financial statement reporting forms. The monthly reporting form has not been changed (Version 6.0.0).
The reporting forms (Version 8.0.0) dated December 8, 2011 are required for any new submissions of the quarterly or annual financial statements; and are to be used for revising or making changes to previously filed statements. The only change made to the form is the version number which is now Version 8.0.0, the change was made to update our electronic filing process.
The revised reporting forms, Version 8.0.0 dated December 8, 2011, is effective immediately.

Use the financial statements shown below, implemented on 12/31/2008, for annual, quarterly and monthly reports.

New Health Plan Financial Statement Reporting Application
The Plan's Financial Statement web portal single login/password will no longer be
active after December 31, 2008. An individual login/password is now required in order
to access the Health Plan Financial Statement Reporting application.
If you require access to the Health Plan Financial Statement Reporting application and:

You (or any other Plan representative(s)) currently have login/password access to the
DMHC web portal for one of the following listed applications, then send an email only to
Licensing eFiling (LicensingeFiling@dmhc.ca.gov) requesting access
to the Health Plan Financial Statement Reporting application using your same assigned
individual access information. Current web portal applications: eFiling, Quarterly Grievance
Reports, Financial Solvency Reporting, and Claims Settlement Practices and Dispute Resolution

You currently DO NOT have login/password access to the DMHC web portal for any of the
above listed applications, please submit the
Electronic Filing Signature Verification Form,
checking the box that indicates HEALTH PLAN FINANCIAL STATEMENT REPORTING.
Please complete, print, sign (both individual and authorization signatures), and mail the original
hardcopy to the Department for processing. The individual login/password access
information will be mailed directly to the individual.
The major changes in the new system that affect health plans are:
- Users will now have unique logins. Your login will be a combination of the health plan name and
a new login and password that will be assigned to each individual.
- Reports no longer need to be submitted to DMHC all at once. Users will be able to start a report and
save their information if they need to return at a later date to complete the report.
- Users will be able to view a history of all the reports that have been submitted.
- Users will be able to view the status of a report and who the filing is assigned to at DMHC.
- New annual, quarterly and monthly reporting forms.
